Egypt's Mubarak on life support after heart stops

CAIRO (AP) ? Egypt's Hosni Mubarak was being kept alive by life support after he was rushed from prison to a military hospital in a rapidly worsening condition, officials said. The 84-year-old ousted leader's health crisis added a new element of uncertainty just as a potentially explosive fight opened over who will succeed him.

The state news agency MENA said Mubarak was "clinically dead" when he arrived at the hospital and that doctors used a defibrillator on him several times. It initially said the efforts were not successful.

But the official said Mubarak was put on life support. He had no further details on his condition. The official spoke on condition of anonymity because he was not authorized to talk to the press.

The developments add further layers to what is threatening to become a new chapter of unrest and political power struggles in Egypt, 16 months after Mubarak was ousted by a popular uprising demanding democracy. Egyptians were uncertain about Mubarak's fate, about who will succeed him and about whether his successor will have any power.

The campaign of Mubarak's former prime minister, Ahmed Shafiq, said Tuesday he has won Egypt's presidential election, countering the Muslim Brotherhood's claim of victory for its candidate, Mohammed Morsi.

The election commission is to announce the official final results on Thursday and no matter who it names as victor, his rival is likely to reject the result as a fraud. If Shafiq is declared winner in particular, it could spark an explosive backlash from the Brotherhood.

The Brotherhood, Egypt's most powerful political group, is already escalating its challenge against the ruling military over the generals' move this week to give themselves overwhelming authority over the next president. Some 50,000 protesters, mostly Islamists, massed in Cairo's Tahrir Square on Tuesday evening chanting slogans in support of Morsi and denouncing the generals' power grab.

The health crisis of Mubarak, who is serving a life prison sentence, is yet one more thing to stoke the heat.

Moving Mubarak out of prison is likely to further infuriate many in the public. Many Egyptians have been skeptical of earlier reports that his health was worsening since he was put in prison on June 2, believing the reports were just a pretext to move him to another facility. There is a widespread suspicion that security and military officials sympathetic to their old boss are giving him preferential treatment.

Details of the crisis were still sketchy. Earlier the news agency and officials said that while at the Torah Prison hospital he suffered a "fast deterioration of his health." His heart stopped beating until he was revived by defibrillation, then he suffered a stroke.

At that point, he was moved from the prison hospital to Maadi military hospital ? notably the same one where his predecessor Anwar Sadat was declared dead more than 30 years ago after being gunned down by Islamic militants. That was when MENA reported him "clinically dead."

The criteria for using that term are "poorly defined," said said Dr. Lance Becker, a University of Pennsylvania emergency medicine specialist and an American Heart Association spokesman. "In its crudest form, clinical death just means that a doctor thinks he's dead ? somebody standing at the bedside believes he is dead," he said.

"My speculation would be that he had that sort of event where his heart temporarily stopped," said Becker, who is not involved in Mubarak's treatment. "That doesn't mean that it's irreversible," and life support can be used to keep his blood circulating and replace breathing if he is unable to do so on his own, Becker said.

Mubarak's condition brought to mind former Israeli Prime Minister Ariel Sharon ? though it was not known if there was any medical similarity in their conditions. Sharon suffered a massive stroke on 2006. Intensive treatment and repeated operations by a team of brain surgeons stabilized his condition, but he never regained consciousness. Sharon, 84, is still alive but remains on life support in a deep coma.

Mubarak has been serving a life sentence at Cairo's Torah Prison for failing to stop the killing of protesters during the 18-day uprising against his rule last year. The verdict against him has already been a spark for protests ? thousands massed in Tahrir when the court acquitted him and his sons on separate corruption charges and cleared several top security chiefs on the protester killings.

The multiple disputes have turned a moment that was once anticipated by some as a landmark in Egypt's post-Mubarak transition ? the election of the first civilian president in 60 years ? into a potentially destabilizing snarl.

Shafiq's campaign spokesman, Ahmed Sarhan, told a televised news conference that Shafiq won 51.5 percent of the vote and that the claim of victory by Morsi was "false."

"Gen. Ahmed Shafiq is the next president of Egypt," said Sarhan. He said Shafiq won some 500,000 votes more than Morsi, of the fundamentalist Brotherhood.

The Shafiq campaign's claim came just hours after Morsi's campaign repeated their claims of victory, saying Morsi had won 52 percent of the vote compared to Shafiq's 48.

The Brotherhood first announced Morsi's victory early Monday, around six hours after polls closed. It said its claim was based on returns announced by election officials from each counting center around the country. Each campaign has representatives at every center, who compile the individual returns. The Brotherhood's compilation during the first round of voting last month proved generally accurate and, when it announced its victory early on in that race, it raised no objections.

But this time, Shafiq's campaign countered quickly, saying early Monday that its ongoing count showed their man ahead. Tuesday's announcement was its first claim that it had won.

Shafiq, a former air force commander who was named prime minister during Mubarak's last days, is seen by his opponents as likely to preserve the military-backed police state that his former boss headed for three decades. He, in turn, has presented himself as a strongman able to keep Egypt stable and out of the hands of the Brotherhood, playing on fears the group will turn the country into an Islamic state.

Just as polls closed on Sunday night, the military ? which has ruled since Mubarak fell on Feb. 11, 2011 ? issued a constitutional declaration that gave themselves power that all but subordinates the new president, a move critics called a "coup" intended to maintain their control over the state even after they nominally transfer authorities to the president by July 1.

The declaration gave the generals legislative powers and control over the process of drafting a new constitution and the national budget. It also shields the military against any kind of civilian oversight and allows the generals to run their own affairs without interference from civilian authorities.

A court ruling also dissolved the Islamist-dominated parliament last week, a verdict that has been endorsed by a decree issued by military ruler Field Marshal Hussein Tantawi. The Brotherhood and its Islamist allies dismissed the decree, arguing that Tantawi had no right to issue it with less than two weeks before the scheduled transfer of power to civilians. Also last week, the military-backed government granted military police and intelligence agents the right to arrest civilians for a host of suspected crimes, a move that many viewed as tantamount to a declaration of martial law.

Thousands were demonstrating in Cairo and the Mediterranean port city of Alexandria Tuesday evening to denounce the constitutional declaration, which also strips the next president of significant powers and the court ruling.

The estimated 50,000 protesters in Cairo's Tahrir Square, birthplace of last year's anti-Mubarak uprising, were mostly Brotherhood supporters and other Islamists joined by a small group of leftist and liberal activists.

"It is not possible to have a revolution and then have military rule and a president with no authority," said protester Mohammed Abdel-Hameed, a 48-year-old schoolmaster who said he came with his son and others to Tahrir from Fayyoum, an oasis province 60 miles (100 kilometers) southwest of Cairo.

"How can they dissolve parliament and spend all that money again on another election? This is a waste of public funds," he said. "If they want blood we are ready to offer blood. I am. So that my son can live free," warned Abdel-Hameed, who wore traditional galabeya robes.

Some protesters gathered outside parliament earlier Tuesday, and a number of lawmakers tried to enter the building but were turned away by security forces. Hundreds of black-clad policemen armed with clubs and shields ringed the building, standing behind metal barricades.

"Wake up, field marshal. We defended our revolution through the ballot boxes," protesters chanted, referring to Tantawi, Mubarak's defense minister for 20 years. "You coward Field Marshal, free parliament," they screamed.

"No revolution that is protected by God will fail," said Saber Ibrahim, a 36-year-old school teacher who came from his native Beni Suef south of Cairo to participate in the rally. "We, the people, gave them (the military) legitimacy and we now are taking back."

On Tuesday former U.S. President Jimmy Carter expressed his concerns. His Carter Center monitored the weekend runoff as it has every nationwide vote in Egypt since Mubarak's ouster in a popular uprising engineered by pro-democracy youth groups.

"I am deeply troubled by the undemocratic turn that Egypt's transition has taken," Carter said in a statement Tuesday. He pointed to the dissolution of parliament and the elements of martial law and said the constitutional declaration "violated the military's commitment to make a full transfer of power to an elected civilian government.

"An unelected military body should not interfere in the constitution drafting process," said Carter, alluding to the military's control over the writing of the charter. Carter is a regular visitor of Egypt, where he often meets with the ruling generals and government leaders.

Amnesty International said the powers acquired by the military could lead to further human rights violations and, unless modified, they would allow the generals "to continue to trample on human rights with impunity."

It said the control of the military over the panel tasked with drafting the constitution allows it to reject any attempt to restrain the military, put it under civilian oversight or hold it accountable for human rights abuses.

Hoda Mahmoud, a 32-year old human resources manager, was out protesting at Tahrir Square on Tuesday. A member of a youth group known as the Revolutionary Socialists, she said her real fight is with the military and predicted that the Brotherhood may eventually back down.

"The fight is in the street, not one about political settlements," she said, alluding to the Brotherhood's reputation for political opportunism and appetite for back-room deals.

Naturally, Tim Cook's crew appealed that decision, and the Court of Appeals for the Federal Circuit (CAFC) has decided to give Apple another crack at obtaining an injunction. The CAFC upheld the lower court's ruling as to three of the four patents, but found fault with the District Court's holding that Apple's tablet design patent had substantial questions of validity.

Essentially, the lower court held that Apple's patent was likely no good because it was an obvious design in light of two tablets that were created long before Apple patented the iPad's look. However, the CAFC found that one of the previous slate's asymmetrical bezel and lack of an unbroken, all-glass surface (among other differences) were sufficient to render Apple's patent non-obvious. Basically, the appellate court found that the District court "construed the claimed design too broadly," and remanded the issue so that the district court could complete its preliminary injunction analysis. So, Apple's cleared a big hurdle towards getting the Galaxy Tab 10.1 off the US market, but the company's still got to persuade Judge Koh that it'll be irreparably harmed without the injunction. This decision assures even longer legal proceedings, but given how well both of these tech titans are doing these days, we're pretty sure they can afford the attorneys' fees.

Washington and NATO stepped up those efforts in recent days by making it clear Islamabad would not be welcome at the two-day summit beginning Sunday in Chicago unless it did so.

NATO Secretary-General Anders Fogh Rasmussen phoned President Asif Ali Zardari on Tuesday afternoon to invite him to the meeting, according to a statement from the Pakistan government and NATO.

"'This meeting will underline the strong commitment of the international community to the people of Afghanistan and to its future,' NATO spokeswoman Oana Lungescu said in Brussels, where the alliance is based. 'Pakistan has an important role to play in that future.'"

The AFP reports that Pakistan has all but accepted the offer and this marks a "return from the cold for Islamabad, which boycotted the last major international talks on Afghanistan, held in Bonn in December."

The wire service reports that a cabinet meeting on Wednesday is widely expected to approve the reopening of the supply routes.

ScienceDaily (May 13, 2012) ? Using tiny solar-panel-like cells surgically placed underneath the retina, scientists at the Stanford University School of Medicine have devised a system that may someday restore sight to people who have lost vision because of certain types of degenerative eye diseases.

This device -- a new type of retinal prosthesis -- involves a specially designed pair of goggles, which are equipped with a miniature camera and a pocket PC that is designed to process the visual data stream. The resulting images would be displayed on a liquid crystal microdisplay embedded in the goggles, similar to what's used in video goggles for gaming. Unlike the regular video goggles, though, the images would be beamed from the LCD using laser pulses of near-infrared light to a photovoltaic silicon chip -- one-third as thin as a strand of hair -- implanted beneath the retina.

Electric currents from the photodiodes on the chip would then trigger signals in the retina, which then flow to the brain, enabling a patient to regain vision.

A study, to be published online May 13 in Nature Photonics, discusses how scientists tested the photovoltaic stimulation using the prosthetic device's diode arrays in rat retinas in vitro and how they elicited electric responses, which are widely accepted indicators of visual activity, from retinal cells . The scientists are now testing the system in live rats, taking both physiological and behavioral measurements, and are hoping to find a sponsor to support tests in humans.

"It works like the solar panels on your roof, converting light into electric current," said Daniel Palanker, PhD, associate professor of ophthalmology and one of the paper's senior authors. "But instead of the current flowing to your refrigerator, it flows into your retina." Palanker is also a member of the Hansen Experimental Physics Laboratory at Stanford and of the interdisciplinary Stanford research program, Bio-X. The study's other senior author is Alexander Sher, PhD, of the Santa Cruz Institute of Particle Physics at UC Santa Cruz; its co-first authors are Keith Mathieson, PhD, a visiting scholar in Palanker's lab, and James Loudin, PhD, a postdoctoral scholar. Palanker and Loudin jointly conceived and designed the prosthesis system and the photovoltaic arrays.

There are several other retinal prostheses being developed, and at least two of them are in clinical trials. A device made by the Los Angeles-based company Second Sight was approved in April for use in Europe, and another prosthesis-maker, a German company called Retina Implant AG, announced earlier this month results from its clinical testing in Europe.

Unlike these other devices -- which require coils, cables or antennas inside the eye to deliver power and information to the retinal implant -- the Stanford device uses near-infrared light to transmit images, thereby avoiding any need for wires and cables, and making the device thin and easily implantable.

"The current implants are very bulky, and the surgery to place the intraocular wiring for receiving, processing and power is difficult," Palanker said. The device developed by his team, he noted, has virtually all of the hardware incorporated externally into the goggles. "The surgeon needs only to create a small pocket beneath the retina and then slip the photovoltaic cells inside it." What's more, one can tile these photovoltaic cells in larger numbers inside the eye to provide a wider field of view than the other systems can offer, he added.

Stanford University holds patents on two technologies used in the system, and Palanker and colleagues would receive royalties from the licensing of these patents.

The proposed prosthesis is intended to help people suffering from retinal degenerative diseases, such as age-related macular degeneration and retinitis pigmentosa. The former is the foremost cause of vision loss in North America, and the latter causes an estimated 1.5 million people worldwide to lose sight, according to the nonprofit group Foundation Fighting Blindness. In these diseases, the retina's photoreceptor cells slowly degenerate, ultimately leading to blindness. But the inner retinal neurons that normally transmit signals from the photoreceptors to the brain are largely unscathed. Retinal prostheses are based on the idea that there are other ways to stimulate those neurons.

The Stanford device uses near-infrared light, which has longer wavelength than normal visible light. It's necessary to use such an approach because people blinded by retinal degenerative diseases still have photoreceptor cells, which continue to be sensitive to visible light. "To make this work, we have to deliver a lot more light than normal vision would require," said Palanker. "And if we used visible light, it would be painfully bright." Near-infrared light isn't visible to the naked eye, though it is "visible" to the diodes that are implanted as part of this prosthetic system, he said.

Palanker explained what he's done by comparing the eye to camera, in which the retina is the film or the digital chip, and each photoreceptor is a pixel. "In our model we replace those photoreceptors with photosensitive diodes," he said. "Every pixel is like a little solar cell; you send light, then you get current and that current stimulates neurons in the inner nuclear layer of the retina." That, in turn, should have a cascade effect, activating the ganglion cells on the outer layer of the retina, which send the visual information to the brain that allows us to see.

For this study, Palanker and his team fabricated a chip about the size of a pencil point that contains hundreds of these light-sensitive diodes. To test how these chips responded, the researchers used retinas from both normal rats and blind rats that serve as models of retinal degenerative disease. The scientists placed an array of photodiodes beneath the retinas and placed a multi-electrode array above the layer of ganglion cells to gauge their activity. The scientists then sent pulses of light, both visible and near-infrared, to produce electric current in the photodiodes and measured the response in the outer layer of the retinas.

In the normal rats, the ganglions were stimulated, as expected, by the normal visible light, but they also presented a similar response to the near-infrared light: That's confirmation that the diodes were triggering neural activity.

In the degenerative rat retinas, the normal light elicited little response, but the near-infrared light prompted strong spikes in activity roughly similar to what occurred in the normal rat retinas. "They didn't respond to normal light, but they did to infrared," said Palanker. "This way the sight is restored with our system." He noted that the degenerated rat retinas required greater amounts of near-infrared light to achieve the same level of activity as the normal rat retinas.

While there was concern that exposure to such doses of near-infrared light could cause the tissue to heat up, the study found that the irradiation was still one-hundredth of the established ocular safety limit.

Since completing the study, Palanker and his colleagues have implanted the photodiodes in rats' eyes and been observing and measuring their effect for the last six months. He said preliminary data indicates that the visual signals are reaching the brain in normal and in blind rats, though the study is still under way.

While this and other devices could help people to regain some sight, the current technologies do not allow people to see color, and the resulting vision is far from normal, Palanker said.

The PTA arrives in Mexico's schools

Parent-teacher associations are the face of the US public school system, but it is only recently that parents are starting to penetrate the hallways of Mexico's schools.

When Karina Salda?a enrolled her first child in elementary school, she hoped for the kind of parent participation that didn't exist when she was growing up. Her own school director was ?untouchable? to both teachers and students alike, Ms. Salda?a says.

But what she found was the same impenetrable wall that has long kept parents locked out of the public education system in Mexico ? where in some schools teachers don't show up to class or are woefully underqualified, and where students drop out at high rates.

A decade later, however, with her second child now in sixth grade at the same school, Salda?a is busy most weeks at the Fray Matias de Cordova elementary school.? Located on a busy block in central Tuxtla Gutierrez, the capital of Chiapas, Salda?a is the parents' representative on the official parent teacher council. ?There has been a cultural shift,? she says. ?They have involved us.?

Parents associations are the face of the US public school system. But in Mexico, although by law they have existed for years, it is only recently that parents are starting to penetrate the hallways of Mexico's schools, not only helping to build infrastructure or raise money, but to demand that teachers show up to class and equip pupils with the basic skills they need to advance. And new research shows that such participation is having a positive impact on dropout rates and even test scores.

?Societal consciousness has grown about the necessity of including parents,? says Dolores Ramirez, the head of a program called School Management Support (AGE) under Mexico's Ministry of Education that funds rural parents associations. ?Before, parents dropped their kids off and handed the responsibility of education to the school.?

For decades, parents in Mexico were disregarded. Many accepted a secondary role, deferring to teachers and principals. This is especially true in rural communities, where the schoolteacher is often the most educated member of the community and parents work long days, often with the help of their children. Many teachers once reckoned that schools were better off without parental interference.

Mexico's efforts to reinforce parental roles in schools was part of the school decentralization movement that gained traction across the globe in the 1990s from New Zealand to the Netherlands, and Honduras to Hong Kong. After decentralizing its system in 1992, Mexico's Ministry of Education gave new support to programs to help parents in rural and marginalized schools and later instituted councils of teachers, parents, and directors to help oversee compensatory funds.

?We thought it was better to keep them far away from the school,? says Marilu Sarmiento, the director of Fray Matias de Cordova, who was a teacher for 16 years before moving into administration. ?[Now], each day parents participate more.?

Ms. Sarmiento?s school bustles on a recent morning. One class is doing calisthenics, not far from the new tables Ms. Salda?a's PTA constructed in December. They remodeled a cafeteria in August. Now they are planning to build a bigger boys' bathroom and launch a general cleaning campaign. They also began a program in February called ?Return to School,? where parents lead the Monday school ritual of singing national and state hymns, and raising the Mexican flag.

Their work is voluntary and unpaid. In rural schools, parent participation has grown with the help of AGE, which began in 1996 in 5,200 schools, and has since expanded to 45,000 programs throughout Mexico today, says Ms. Ramirez.

Particularly active in large, rural states such as Chiapas, the AGE program gives small amounts of money, anywhere from $300 to $700 a year, to the school's parent association, to plan projects like constructing bathrooms, painting buildings, or buying vital school supplies such as pencils. The program is an important resource, as school spending in the budget, which is generous 22 percent of total government expenditure, goes almost entirely to salaries, says Harry Patrinos, the lead education economist at the World Bank.

First, Melissa is a horse-face, gold digging tramp. Caroline is boring. Rosie's a butchy boy, and the idiot cousin (I can only assume he means Kathy ... no offense to Kathy) looks like a frog. 

Joe Guidice certainly has an opinion about everyone but the scariest is the one he holds about himself. He tells Teresa he's the powerhouse and she doesn't need anyone else. Minus 12. What happens when they haul his sorry butt off to jail again?

Oh, and Teresa says her brother is a little pansy for telling his wife about their conversation. How can you not love family?

But Teresa's not just sitting around bad mouthing her family. She's off to Jacqueline's for some gossip and a session with personal trainer Jolene.
